Policies and Procedures for

Online Personal Training and Nutrition 

Payment Policies:

When you sign up for Shape It Up, LLC (SIU) through PayPal, your account will become active when payment is received.  PayPal payments are on a monthly or yearly recurring basis. 

Cancellations:

To cancel your membership, please email a statement that you wish to terminate your agreement.  You must give 30 days advance notice of termination or account will be billed for normal payment amount. Memberships will not be prorated for any reasons. SIU is not responsible for terminating PayPal recurring charges.  Please, go to PayPal site and terminate your recurring charges. You can access this through the Shape It Up website by clicking on “Cancel Membership”.

Confidentiality:

Please note:  All information given for your online personal training and nutrition will be kept confidential and will only be shared with your written consent.  TrainerForce (the host Member's Only website company) has their own policy and procedure forms online which you must agree to before SIU membership is complete.  Be aware that you are providing your information via internet and as with any site there are risks to identity theft, etc.  If you wish not to provide your information via internet, a packet can be mailed to you to fill out and return. (Return postage at client’s expense.) 

 Refund Policy:

 Refunds will only be made under the following condition:

A client’s medical condition exists that makes it impossible to utilize a fitness or nutritional program .  In this case, a physician’s written notification is required. The refund will start when physician’s note has been received by Shape It Up, LLC.

 

Waiver of Liability and Informed Consent: 

I,    (Insert Client’s Name):  (from here out referred to as the Client), have enrolled in a program of strenuous physical activity including, but not limited to, strength, cardiovascular training and flexibility exercise either in a fitness facility, outdoor location or in-home setting.  I hereby affirm that I am in good physical condition and do not suffer from any disability that would prevent or limit my participation in this fitness program and have received a doctor’s consent to exercise.

 I, the Client,  understand that this is program was designed for me and will not provide or share SIU information to other individuals.

I, the Client, attest that I am over 18 years old.

 I, the Client,  agree to allow SIU to use any photos submitted or any verbal statements for the use of promotions or other business related agendas.

 In consideration of my participation in personal training/ lifestyle & weight management with Shape It Up, LLC,

I, the Client, for myself, my heirs and assigns, hereby release Shape It Up, LLC  , any affiliates, and family members from any claims, demands and causes of action arising from my participation in this fitness program.

 I, the Client, fully understand that I may injure myself as a result of my participation in Shape It Up, LLC  online personal training and nutrition program and I hereby release Shape It Up, LLC  , any affiliates, and family members from any liability, any negligent act or omission now or in the future including, but not limited to, heart attacks, muscle strains, pulls or tears, broken bones, shin splints, heat prostration, knee/lower back/ foot injuries, and any other illness, soreness, injury, or even death, however caused, occurring during or after my participation in this fitness/nutritional program.

 I, the Client, hereby acknowledge the aforementioned has been read carefully and I fully understand the content above. Any questions that may have occurred to me concerning this document have been answered to my satisfaction.
